Is your organization changing its name or moving to a new location?

Please complete the following document for each API Monogram License you currently hold:

Monogram License Agreement

If you are moving to a new location, please include the following information with your correspondence:

1. What is the distance from the old facility location to the new facility location?
2. What is the approximate percentage of employees transferring to the new facility?
3. Will the same Quality Management System be used at the new facility?

Once you have completed the forms, please fax them to the API at 202-682-8070.
